Catholic Charities, is seeking a compassionate and mission driven person, to join our Lead Program at St. Martin de Porres Family Center as a Human Services Worker. In this position you will have the opportunity to work with families with children five (5) years of age and under who are affected by childhood lead poisoning in the Glenville neighborhood. We are looking for experienced, compassionate, and energetic individuals to determine and enroll eligible children and families into the City of Cleveland Lead Hazard Reduction Program.


The ideal candidate must have strong organizational and data management skills; are knowledgeable of working with lead poisoning children and families and/or at -risk families; and have excellent communicate skills both oral and written.


Program Statement:  At St. Martin de Porres Family Center, we provide an array of services from early childhood developmental home-visiting services, older adults program, after-school programming, food pantry, home-delivered meal programs, community-based services for at-risk families, and emergency assistance. St. Martin de Porres is located in the heart of the Glenville community and takes pride in meeting the needs of the residence we serve through service coordination and empowering them to be self sufficient.


What will you do:



  • Determine program eligibility

  • Maintain an active caseload of 40 children and families per year.

  • Responsible for collecting and maintaining enrollment information

  • Staff will be required to obtain all signatures in accordance with the City of Cleveland's LHRP guidelines.

  • Perform data entry tasks daily  

  • Submit all required paperwork (pre-enrollment applications, worksheets, and all client supporting documentation to the City of Cleveland/ LHRP) by the 10th of every month.

  • Complete monthly reports.  

  • Conduct home visits as required.

  • Accompany participants on program outings as needed.

  • Perform other administrative duties as assigned.
