Outreach Recovery is seeking a Medication Assisted Treatment/Psychiatric Nurse Practitioner provider to join our Addiction and Mental Health team in New Jersey with some coverage in Palm Beach County, Florida via telemedicine. This position will task the provider with seeing patients in a medically-assisted recovery program. Working with our Addiction Team the Provider delivers medical care for specific patients, including formulating a diagnosis and a treatment plan and providing continued follow up patient care.
- Provide direct primary and acute addiction care services to adult patients
- Perform initial and on-going assessment of patients' medical, physical and psychosocial status
- Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history
- Identify medical and health risks
- Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
- Formulate appropriate differential diagnosis based on history, physical examination and diagnostic testing
- Identify needs of the individual, family or community and develop a plan of care and/or education plan
- Make appropriate referrals to other health professionals and community agencies
- Reassess and modify the plan as necessary to achieve medical and health goals
- Contribute to continuing education, expert blogs, and ongoing outreach marketing activities
- Develop a patient education plan relevant to the condition and patient/ family needs
- Participate in performance improvement activities as assigned
Qualifications:
- Degree as a Psychiatric Nurse Practitioner; valid New Jersey NP license (Nurse Practitioner License); CNS (Clinical Nurse Specialist) in Adult Psychiatry / mental health with substance abuse experience; computer proficiency. Wellness and Recovery orientation.
- Must be waivered for 100+ patients for Buprenorphine treatment.

Pay: $130,000.00 - $165,000.00 per year
